FindMyInjuryDoctor.com (“we,” “us,” “our”) is a free directory service operated by Practice Marketing Guru LLC d/b/a FindMyInjuryDoctor.com, [BUSINESS ADDRESS]. We help people injured at work, in motor vehicle collisions, and in slip-and-fall incidents locate medical providers experienced in injury care.
We are not a medical provider, a medical group, an insurance company, or a law firm. We do not provide medical treatment, medical advice, diagnosis, or legal advice. We are not affiliated with the U.S. Department of Labor, the Office of Workers’ Compensation Programs, or any government agency.
This policy explains what we collect when you use this website or submit our search form, how we use it, and who we share it with.
2. Summary
The short version
- When you complete our form, we share the contact details you give us with the participating clinics we match you to, so they can contact you about care. That sharing is the service you asked for.
- We are paid by the clinics that participate in our directory. That is how the service stays free to you.
- We do not share or sell mobile phone numbers or SMS opt-in data with third parties or affiliates for marketing or promotional purposes.
- You can opt out of our text messages at any time by replying STOP.
- Depending on where you live, you may have the right to access, delete, or restrict the sale or sharing of your personal information.
3. What we collect
Information you give us
Through our search form and any follow-up communication, we may collect your name, email address, mobile telephone number, ZIP code or city, the type of injury or claim you are dealing with, and any other details you choose to provide.
Information collected automatically
When you visit the site we may collect your IP address, device and browser type, operating system, referring page, pages viewed, and the dates and times of your visits, through cookies and similar technologies.
Information from other sources
We may receive information about you from advertising platforms, analytics providers, and participating clinics — for example, confirmation that an appointment was scheduled.

- To match you with participating providers in your area and to operate the directory
- To contact you about your request, including by email, phone call, or text message where you have consented
- To respond to questions and provide customer support
- To measure and improve the site, our advertising, and our services
- To detect and prevent fraud, abuse, and security incidents
- To comply with legal obligations and to establish or defend legal claims
5. How we share it
Participating providers
When you submit our form and consent to be contacted, we share the contact information and injury details you provided with one or more participating clinics or medical providers so they can contact you about treatment. This is the core purpose of the service and the reason the form exists.
We are compensated by participating providers. Providers pay us a fixed fee to participate in the directory. Inclusion in our directory is not an endorsement, a credential, a certification, or a guarantee of quality, availability, or outcome, and you should independently satisfy yourself about any provider before beginning treatment.
Once your information is shared with a provider, that provider’s own privacy policy governs how it handles your information. Each provider is independently responsible for obtaining any consent it needs before contacting you, including for any text messages it sends.
Service providers
We share information with vendors who perform services for us — form and hosting providers, communications platforms, email delivery, analytics, and customer support tools — under agreements that limit their use of your information to providing those services.
Legal and safety
We may disclose information where we believe in good faith it is required by law, subpoena, or legal process, or where necessary to protect the rights, property, or safety of any person.
Business transfers
If we are involved in a merger, acquisition, financing, or sale of assets, your information may be transferred as part of that transaction.
Sale or sharing for advertising
Some state privacy laws define “sale” and “sharing” broadly. Our disclosure of your information to participating providers in exchange for compensation, and our use of advertising cookies, may be treated as a sale or sharing under those laws. See State privacy rights for how to opt out.
6. Mobile and SMS data
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Mobile opt-in data and consent to receive text messages from FindMyInjuryDoctor.com are never sold, rented, or shared with any third party, affiliate, or other organization for marketing or promotional purposes.
Information sharing with subcontractors that support our messaging platform — for example, a communications provider that transmits the messages on our behalf — is permitted solely for that purpose and for no other use.
Your consent to receive text messages from us is specific to FindMyInjuryDoctor.com and is not transferable. It does not authorise any other business, provider, or affiliate to text you. If a participating provider wishes to send you text messages, that provider must obtain your consent separately and directly.
You may opt out of our text messages at any time by replying STOP to any message. Reply HELP for assistance. See our SMS program terms for full details, including message frequency and rates.
7. Consumer health data
Some of the information you provide — such as the nature of your injury or the type of care you are seeking — may be considered “consumer health data” under laws including the Washington My Health My Data Act and Nevada’s consumer health data law.

8. Cookies and tracking
We use cookies, pixels, and similar technologies to operate the site, remember your preferences, measure performance, and support advertising. Advertising partners may use these technologies to show you ads on other sites.
You can control cookies through your browser settings. Blocking some cookies may affect how the site works. We honour the Global Privacy Control (GPC) signal where required by law.
9. Your choices
- Text messages: reply STOP to any message from us.
- Email: use the unsubscribe link in any marketing email.
- Phone calls: tell us, or tell the provider contacting you, that you do not wish to be called again.
- Advertising cookies: adjust your browser settings or use an opt-out preference signal.
- Deletion: contact us using the details in section 15 and we will delete your information subject to any legal retention obligations.
Opting out of marketing does not stop transactional messages relating to a request you have already made.
10. State privacy rights
Depending on your state of residence, you may have the right to:
- Know what personal information we have collected about you and how we use and disclose it
- Request a copy of that information, or have it transferred to another service
- Request correction of inaccurate information
- Request deletion of your information
- Opt out of the sale or sharing of your personal information, and of targeted advertising
- Not be discriminated against for exercising these rights
- Appeal a decision we make about your request
To exercise any of these rights, contact us using the details in section 15. We will verify your request before acting on it, and we will respond within the time frame required by applicable law. An authorised agent may submit a request on your behalf with proof of authorisation.
California residents: we disclose personal information to participating providers for compensation, which California law may treat as a sale. You may opt out by contacting us or by enabling a browser opt-out preference signal. We do not knowingly sell or share the personal information of consumers under 16.
11. Retention
We keep the information you submit for as long as needed to provide the service, meet legal and recordkeeping obligations, resolve disputes, and enforce our agreements. Records evidencing consent to be contacted are retained for the period required to demonstrate compliance with applicable communications laws. When information is no longer needed, we delete or de-identify it.
12. Security
We use administrative, technical, and physical safeguards designed to protect your information. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
13. Children
This site is not directed to children under 13, and we do not knowingly collect personal information from them. If you believe a child has provided us information, contact us and we will delete it.
14. Changes
We may update this policy. We will change the effective date at the top and, where the change is material, provide additional notice. Continued use of the site after an update means you accept the revised policy.
